POSITION: Counseling Testing Referral (CTR) Specialist

QUALIFICATIONS/EDUCATION:

  • Associates of Arts or Science preferred.
  • Reliable transportation for work related travel.
  • The candidate must possess superior organizational, administrative, time management, interpersonal and communication skills.

Responsible for:

  • Obtain informed consent from clients for HIV testing
  • Provide pre-test and post-test counseling to clients  
  • Administer oral HIV test to clients who have consented
  • Identify and discuss clients sexual risk behaviors
  • Coordinate and track internal/external referrals given to clients  
  • Complete CTR certification course.
  • Collect and enter client data into assigned database. 
  • Maintain complete and keep up-to-date client record documentation.
  • Conduct CTR services in community settings and other venues.
  • All other duties as assigned

GENERAL JOB RESPONSIBILITES:

  • Willingness to work flexible hours, including evening and weekend shifts if needed
  • Proficiency with standard office software (Microsoft Word, Excel, and PowerPoint as well as Internet applications) and the ability to learn new computer applications.
  • Ability to work both independently and as part of a team

  • Interest in public health, health policy, and/or medicine

POSITION: Social Network Strategy Coordinator/ HIV Counselor

QUALIFICATIONS/EDUCATION:
• Associates of Arts or Science preferred.

• Clean criminal record.
• Fluency in both English and Spanish preferred

Responsible For:
Providing prevention services that target high risk individuals and fostering trusting relationships and social connections in that community. Developing strategies to access hidden and hard to reach populations, who may be infected but are unaware of their status. Maintain documentation of services and staff activities through computer software, such as; internet pod-casting, Program Monitoring & Evaluation System database, PowerPoint, and Word. The candidate must possess superior organizational, administrative, time management, interpersonal and communication skills, with the ability to interact professionally at all levels, including direct clientele interaction.

GENERAL JOB RESPONSIBILITIES
1. Conduct group and individual orientation with potential SNS recruiters.
2. Perform Testing & Counseling referral services
3. Develop publicity campaign and promotional material for program
4. Make appropriate referrals and initiates consultations with Miracle of Love’s various departments and/or other area agencies.
5. Maintain all required and appropriate documentation in accordance with company policy.
6. Report all program data to direct supervisor
7. Meet service productivity (direct client services) standards as defined by company policy.
8. Cover on-call assignments and provide back-up coverage as needed.
9. Data entry into assigned database for record keeping purposes.
10. Attend and participate in staff meetings and events as required.

POSITION: Ryan White Case Manager (located in Osceola Office)

QUALIFICATIONS/EDUCATION:
• Associate’s degree in nursing acceptable.
• Bachelor’s degree required in social science or health field or Bachelor’s degree in a field other than social science or health and have a minimum of one year of case management experience.
• A valid driver’s license and clean record.

Responsible For:
Providing case management services; including assessing consumer’s needs, conducting psychosocial evaluations, developing individual treatment plans, maintaining documentation of services, and providing education and support to consumers and their immediate family members. Making appropriate referrals and consultations both within and outside the agency. Monitoring all aspects of care. Preparing and maintaining client records.

GENERAL JOB RESPONSIBILITIES
1. Provide case management services for assigned clients including assessment, planning, linking, monitoring and advocacy.
2. Maintain all required and appropriate documentation for client records and progress notes for assigned cases in accordance with company policy.
3. Monitor client participation in planned services.
4. Initiate revisions to treatment plan/plan of care or the closing/transferring of cases in accordance with company procedures.
5. Meet service productivity (direct client services) standards as defined by company policy.
6. Completes on-call assignments as needed.
7. Data entry into assigned database for record keeping purposes.
8. Attend and participate in staff meetings and events as required.
